Supported Health Care Decision-Making

Like all of us, people with disabilities may need support to understand health information, communicate with their clinicians and manage their daily health needs. Using Supported Health Care Decision-Making, adults with disabilities can name trusted supporters to help them access health care, understand medical information and make medical decisions.
